Damaged Passports
Passports or passport cards that are damaged or in poor condition should be replaced, regardless of remaining validity. For security reasons, damaged passports may be retained by the Embassy. It takes about two weeks for the new passport to arrive.

Application Requirements
Please bring the following with you to the Embassy to apply for your replacement passport:
your damaged U.S. passport
proof of your citizenship and identity (see below)
the fee of US$100 if the applicant is age 16 or over, or US$85 if the applicant is under age 16, payable in either Dollars or Euros, using cash or credit card
Proof of U.S. citizenship, such as:
original U.S. Birth Certificate; or
original U.S. Certificate of Citizenship; or
original U.S. Consular Report of Birth Abroad; or
original U.S. Naturalization Certificate; or
a previous expired U.S. passport.
If your proof of U.S. citizenship is one of the first three choices above, then please also bring proof of identity, such as:
a valid passport of any country; or
a valid driver's license with photograph; or
a national identity card with photograph; or
a valid U.S. or State government ID card; or
a U.S. military ID card; or
an identifying witness.
This last choice is the option of last resort, only to be used if no other forms of identification are available. The identifying witness can be a friend or relative who has known you for over two years and who can present a valid passport (preferably U.S. passport) and who is able to submit a sworn statement confirming your identity.
If you are applying for a replacement passport for child under the age of 16, you will also be required to follow additional instructions.
If you are traveling within two weeks and can provide evidence of this travel, you may be eligible for an emergency limited-validity passport.
